Obtaining Student Transcripts
For MS and PhD students:
A transcript, an official copy of the record of a student's academic career at Dartmouth, lists a student's name class, and all courses taken and grades received by the Registrar's Office. The Graduate Studies Office will, UPON WRITTEN request from students, prepare, mark with the official seal, mail transcripts and citations. It is recommended that students order transcripts at least on week in advance of the required mailing date. There is a charge of $3.00 for each transcript, payable by cash or check. Please send your WRITTEN REQUEST and PAYMENT to:

For MPH students:
A transcript, an official copy of the record of a student's academic career at Dartmouth, lists a student's name class, and all courses taken and grades received by the Registrar's Office. To request a transcript please include the following information in a letter:
- Name and Signature
- Social Security number
- Number of transcripts requested
- Address (s) to mail transcript
Generally, transcripts are sent out within 1 week of receipt of your request.
Requests for MPH transcripts should be forwarded to the Dartmouth Medical School Registrar's Office at the address listed below:
